If more surgeons used this consent form there would be a LOT LESS LASIK going on and substantially reduced risk for OMIC (Ophthalmic Mutual Insurance Company).

Perhaps OMIC should force all surgeons who are covered by OMIC to use their documents. Their list is not complete... the members of The Flap could help flesh it out a bit

You, the doctor, agree that if you wrongly tell me I am an acceptable candidate or otherwise commit malpractice I have the right to castrate you without anesthesia. Or, how about this: You, the doctor agree that if your stupidity or negligence gives me night vision problems I have the right to inflict the same on you by any means necessary?
